Our client is one of the largest and most dynamic local authorities in the UK and is undergoing significant transformational change to further enhance customer focus, service and delivery. The adult operations function delivers direct provision of services relating to vulnerable adults and their families. These services are delivered against specifications set by the people commissioning function in line with the County Council's overarching outcomes. This role reports to an operational team manager and is responsible for facilitating and supporting service users to undertake supported self-assessments and reviews to identify needs and outcomes. Also providing assessment of need taking into account current support systems and to identify potential risks, for example in relation to the mental capacity act, continuing health care and safeguarding. The Role:
Implement each individuals care plan and programme, and provide support to individuals to access occupation, interests, healthcare facilities and other such areas to achieve the outcomes identified within the assessment process and care plan. Job Purpose:
Key Accountabilities:To provide personal support and to assist, teach and encourage service users' personal hygiene skills such as, skincare, bathing, hair washing, ensuring the dignity of the service user is respected at all times.Demonstrating skills in enabling and teaching individuals' to achieve measurable outcomes designed to improve their lifestyles by ensuring individuals' have maximum control and choice in respect of their own livesTo support individuals to achieve the optimum of independence in daily living skills such as shopping, cooking, cleaning and in liaison with the service user plan delivering any necessary additional practical support.Adhere to appropriate County policies, procedures and relevant legislation.Complying with the required standards and policies of the service.Working flexibly to meet the needs of the service users which requires working in a variety of different environments.Contributing to Care Plans, assessments, risk assessments and maintaining accurate and factual records in respect of service users progress.Participating in team meetings and other developmental opportunities as required by the service.To work a flexible rota system over 7 days per week including Bank Holidays.Fitness to undertake the full range of duties including the ability to push a wheel chair if needed by a service user to access the community.
