Our Older Adults Uttlesford Social Care Team is a wonderful team of social work, Occupational Therapists and Assistant Practitioners, working across North and South Uttlesford. We work from our main base in Harlow, but have a number of social and health care based offices within Uttlesford, which we use to maximize flexible working.We are looking for self-directed and passionate social workers to add to our 2 teams in Uttlesford. They will have to be proactive, responsive, enabling and enjoy working in a fast changing environment, where we are introducing different initiatives and new ways of working, in line with the Care Act 2014. Our teams are dedicated to new approaches, prevention and delay of needs and the wellbeing and independence of older adults in Uttlesford. We are working very closely with Health and other partners in our initiatives. There are excellent opportunities for our practitioners for continuous professional development, especially for AMPH, BIA assessor and Practice Educator awards. The Older People Team The Older People Community Teams work with individuals aged 65 and over. Join us, and you'll be fully supported on our multi-disciplinary team and, together, we'll deliver social care as it should be. Here, you'll have the time to undertake social care community assessments, statutory reviews and mental capacity assessments while promoting the use of re-ablement and telecare. Your role will involve attending multidisciplinary meetings, undertaking safeguarding investigations, completing carers' assessments and providing advice and information. You will also complete social care intervention for those cases where the need has been identified and ongoing intervention is required. You'll benefit from working alongside other health partners, police and voluntary organisations. Skills, knowledge and experience Diploma / Degree in Social Work HCPC registered Case load management in line with the Care Act Excellent organisation skills with the ability to manage your own time Ability to engage and influence with multiple agencies Passionate and driven to support your own continuing personal development (CPD) The role requires you to be mobile throughout a wide operational area including travelling around the county of Essex and beyond, transporting service users and their belongings and to be able to do so in an emergency. Driving licence and car or ability to meet the mobility requirements of the role through other means is essential.
